Once you have made your appointment at the Sandhills Spay Neuter Veterinary Clinic (SNVC), we recommend that you also read and sign the high risk waiver form below. If your pet is at an increased risk for surgery, we will require that you sign this form for us to be able to perform surgery. Having it read and signed at intake keeps us from having to reach you during the day.
